1. Introduction: Holistiq Health Group AG, Lessingstrasse 15, 8002 Zurich (“Holistiq”; hereinafter also referred to as “us”, “we”, “our”) offers online and local support for the healing of chronic diseases, health prevention, and optimization. By using and accessing our services (as defined below), you (hereinafter also referred to as “Patient”) agree to the following Terms and Conditions. Please read them carefully:

To personalize and optimize our services, Holistiq may also utilize suitable data technologies. Holistiq’s services are aimed at individuals:
The services offered by Holistiq are not intended for individuals suffering from acute illnesses and emergencies. Holistiq reserves the right to refuse the provision of services without stating reasons.
3. Our Forms of Service Delivery: Holistiq offers its services as follows:
The duration, costs, and specific scope of our services are determined by the individual offer from Holistiq (as indicated on the website or agreed upon with the relevant specialist). Notably, the lab tests mentioned in the plans are covered by the basic health insurance, after a consultation with our doctor. The costs of the plans therefore cover the health coaching and the food supplements (if applicable) but not the lab tests or medical consultations.
Sessions typically last between 30-90 minutes (subject to variations) and might include some offline work from us to prepare the sessions and work on the specific case (notably to issue the personalized recommendations).
Furthermore, depending on the health plan, Holistiq’s services may also include the delivery of dietary supplements and the conducting or arranging of laboratory tests (in collaboration with selected laboratory partners).
4. Locations of Service Delivery: Our services are generally provided either online or offline at our health center at Lessingstrasse 15, 8002 Zurich, depending on the type of service and the needs of each individual patient. The determination of the location of service delivery is at the discretion of the specialist, taking into account the patient’s preferences. However, telemedicine doctors are only available online. The onsite consultations are just for Complementary Medicine and some Holistic Health Coaching and Nutrition.
5. Our Specialists: Our services are provided by qualified and trained specialists, such as complementary therapists, functional medicine doctors, nutritionists, life and health coaches, employed and selected by Holistiq (the “Specialists”). Laboratory testing services are provided by selected and qualified laboratories.
6. Booking and Rescheduling/Cancellation of Appointments: Booking and cancellation of our services can be done online, by phone, via email, WhatsApp, or in-person visits. Booked appointments are binding only when confirmed by Holistiq in a verifiable written form (electronic is acceptable).
The following payment and cancellation conditions apply:
Payment Conditions:
If a specific health plan is booked and the first 90-minute session has started, the entire health plan becomes due. Patient will then receive a series of maximum 5 invoices to pay corresponding to the online and offline work done within the plan. The sum of these 5 installments is the price of the plan and the corresponding insurance receipt (if applicable) will be attached to the invoice for patient to forward to their basic or complementary insurance.

7. Costs and Payment: The costs for the services are based on the prices offered by Holistiq. Holistiq’s invoices are to be paid within a payment period of ten (10) days (unless otherwise specified on the corresponding invoice). If payment is not made within the payment period, default occurs automatically without further notice, and Holistiq is entitled to charge default interest of 5% per annum without further notification. Patients are also required to pay reminder fees of CHF 15.00 per reminder. Holistiq reserves the right to refuse further services in case of non-payment.
Each patient must verify whether the costs for the services are covered by their individual health insurance policy. Holistiq does not guarantee coverage by health insurance as it depends on the franchises and limits specific to each policy. In the case of Telemedicine, the first 40-minute of consultations are covered by the basic insurance as well as the 30-minute follow-ups – as mentioned on the Telemedicine webpage. Not all lab tests are covered by the basic health insurance.
8.Use of Third-Party Software Applications: For online services or online planning and booking of available appointments with our specialists, Holistiq may use third-party software applications. For the use of such software applications, you may need to agree to the applicable terms of use of the respective third-party provider. Holistiq excludes any liability for the functionality or quality of such third-party software applications.

10. Exclusion of Liability: Holistiq excludes its liability for the services to the extent permitted by law. Please note that Holistiq and its specialists cannot promise any treatment success or guarantee treatment success. In particular, there are no claims against Holistiq or its specialists if treatment does not achieve the desired results or does not meet your expectations. You avail yourself of all services provided by Holistiq and its service partners (e.g., laboratory partners) at your own risk and volition.
Holistiq excludes any liability and warranty regarding the dietary supplements ordered through Holistiq. Holistiq does not guarantee that the dietary supplements meet the specific health requirements of the buyer. The information and products provided do not replace medical advice. Furthermore, Holistiq excludes any liability and warranty regarding the laboratory tests performed by Holistiq or its laboratory partners and the results derived therefrom. Holistiq is not liable, in particular, for errors in conducting laboratory tests, errors in interpreting the results of laboratory tests, and damages caused by laboratory tests.
Please note that, aside from its doctors within the scope of their functional medicine services, Holistiq does not provide medical advice or services. In case of a need for medical assistance, patients should consult a qualified physician rather than a Holistiq coach. The responsibility to consult a physician and all associated consequences lie solely with the patient.

12. Patient Information: Patient information will be provided to you by the responsible specialist before the first session. It explains how our services work and the risks and side effects that may occur.
13. Duration: These Terms and Conditions are valid for the duration of the services provided by Holistiq and booked by a patient. They can be terminated at any time. In case of early termination, Holistiq reserves the right to claim damages (regardless of the provisions regarding cancellation/rescheduling, see Section 6).
14. Applicable Law: These Terms and Conditions are subject to Swiss law. Any disputes arising from the use of our services shall be subject to the exclusive jurisdiction of the courts in Zurich, Switzerland.
15. Amendments to these Terms and Conditions: Holistiq reserves the right to amend these Terms and Conditions at any time. Any changes will be published on our website, and it is your responsibility to regularly review them.
